Summer Camp Required Medical Forms

 


We are thrilled to welcome you to camp this summer!

All camper health history and medical forms must be on file NO later than MAY 31st, to ensure we remain in compliance with our Maryland Department of Health (MDH) camp license.

This year we have a NEW two part filing process – keep scrolling to find out everything you need to know to upload your completed forms:

IMPORTANT INFORMATION FOR ALL CAMPERS:

  • Medical form information MUST be within 365 days of the campers annual well-check up visit, and MUST be signed by a Physician, Physician Assistant, or Nurse Practitioner. 

  • We DO NOT accept paper copies of camper health history forms or medication administration authorization forms. Health Information and forms MUST be entered into your CampBrain account.

  • If your medical information and forms are not submitted by May 31, your camper(s) registrations will be canceled. As a reminder, cancellations occurring after the deadline for any reason, including failure to provide medical documentation are bound by our cancellation and refund policy

  • Campers will NOT be admitted to camp with missing or incomplete medical files.


PART 1: ALL REGISTERED CAMPERS

Camper Health History Information MUST be entered into your CampBrain account.

Instructions for entering your camper health history information in CampBrain:

Log in to your CampBrain account here —> select Summer Camp 2023. Select “view details” and navigate to the medical form section of your account and enter in all camper health history into each designated field. See attached screenshots for reference. Once entered, be sure to save your info, log out, and you’re all set!

After you have completed Part 1, if your child will need to take any form of medication while at camp, please continue on to Part 2.


PART 2: COMPLETE THIS STEP IN ADDITION TO PART 1, IF YOUR CAMPER(S) NEEDs MEDICATION ADMINISTERED DURING CAMP

If your child will need to take ANY FORM OF MEDICATION while at camp, including any prescribed EMERGENCY MEDICATION, you must submit a MEDICATION ADMINISTRATION AUTHORIZATION FORM signed by a PHYSICIAN, PHYSICIAN ASSISTANT, or NURSE PRACTITIONER. 

If you indicated in Part 1 that your child will need medication administered at camp, you will receive email notification with the Medication Administration Authorization form. Instructions for uploading your completed and signed form to your account will be included in the email.

Be sure you have completed Part 1 by March 27th, so that you have enough time to upload your Medication Administration Authorization form before the final submission deadline of May 31.
