Summer Camp Forms

SUMMER CAMP REQUIRED FORMS
AND MEDICATION INFORMATION

Please read below to learn about what is required for your child to attend AWN Summer Camps!


Required forms for everyone:

1) Maryland Health Inventory* (Part 1 & 2)

2) Emergency Contact

3) Off-Site & Photo Permissions

Helpful Tip:


You likely do not need to go to the doctor’s

to retrieve the required medical forms.

Your child’s school will usually have a copy

of the medical form we need.

* We can also accept the DC Universal Health Certificate

Note: Medical forms do not need to be from this calendar year, as long as they are medically up to date and signed by a physician.


CPR CERTIFIED STAFF MEMBERS?

Sunscreen at
Summer Camp?

As required by the Maryland Department of Health,
there will always be at least two staff members onsite who are certified in CPR & First Aid at all times that campers are present.

If you would like your child to use sunscreen
while at camp, please complete this form.
We do not provide sunscreen at Art Works Now;
you will need to provide your own.
Please label your child’s sunscreen clearly with their name.


DOES YOUR CHILD take ANY MEDICINE?

Please see below for critical information and required forms regarding campers and medication.

We will need you to have completed and submitted
the following forms no later than the Monday before
your child’s first day of camp:

- Requires a physician’s signature
If you plan to administer your child’s medication
yourself by visiting daily during camp hours,
we do not need this form completed.

IF YOUR CHILD NEEDS TO TAKE
OVER THE COUNTER MEDICATION
WHILE AT CAMP:

Even though the medicine is not prescribed,
we will still need this form completed by their doctor
in order to administer medication onsite;
We will need you to have completed and submitted
the following forms no later than the Monday before
your child’s first day of camp:

  • Medication Administration Authorization

    - Requires a physician’s signature
    If you plan to administer your child’s medication
    yourself by visiting daily during camp hours,
    we do not need this form completed.

The Medication itself:

  1. Bring the medication with you on your first day of camp:
    - Please allow a few extra minutes for drop off on your child’s first day of camp so that we can review the medication and your forms.

  2. Medication must be inside it’s original box:
    - This is so we can view your child’s name, the prescribed dosage, and the expiration date.

    - The medical forms you have provided will need to match the prescription box .

  3. We will store your child’s medicine in our offices, attached to your child’s forms inside a locked box:

    - As required by the Maryland Department of Health, there will always be at least one staff member onsite who is trained and certified in Medication Administration during camp hours.

    - If we leave the building for any reason, the medication and paperwork will come with us, along with a certified administer.

    - If there is a need to shelter in place, the medication and paperwork will come with us, along with a certified administer.

    - If your child’s paperwork dictates that they can self-carry & self-administer, we will need to review with them where they keep their medicine, and when to be sure to bring it with them.

    - If your child’s paperwork dictates that they can self-administer, their medicine will still be stored and locked, and a certified administer will supervise and log each time the medication is taken.

  4. Take the medication home at the end of your child’s time with us:
    - If your child is returning for another week of camp later in the season, we can hold the medication until then
    - We can not hold onto medication that has been left behind after the child is finished with their camps here, and will be forced to destroy it.


HOW TO TURN IN FORMS:

Please turn in forms as soon as possible.
All forms are due no later than the Monday before your child will begin their first week of camp.
This is so we have ample time to review forms and notify you of any issues.

In Person:

You may drop off your forms to our building at 4800 Rhode Island Ave, Hyattsville, MD 20781
- Our office hours are 10am-6pm, Monday thru Friday.
- Saturday drop offs are acceptable, however we are not always open on Saturdays. Please call before coming.
- Please allot for a few minutes for reviewing your forms

Our number one priority at Art Works Now is safety for all.

ALL Art Works Now staff members and volunteers are vetted with references, as well as state and federal background checks.

Art Works Now Summer Camps is a Licensed Maryland Youth Camp, through the The Maryland Department of Health.

Required forms and policies surrounding them are partially dictated by the body of government that oversees
our Maryland Youth Camp Licensing (The Maryland Department of Health), and partially by Art Works Now itself.
